To save you time at your visit, please print out and sign the Release of Medical Information form. Please also read the Notice of Privacy Practices and sign the Privacy Practices Patient Acknowledgement. Please bring both signed forms with you to your appointment. If you cannot print the forms, we will have all forms available at the office.
