How aesthetic eyelid surgery is performed

Because of individual factors, not everyone will achieve the same results from eyelid surgery. Dr. Yohai will recommend the surgical technique that he feels will attain the best outcome for you.

Improving the appearance of your upper eyelids

For upper eyelid surgery, generally an incision is hidden within the natural fold of the upper eyelid and extends slightly beyond the outer corner into the laugh lines or other existing creases. Through this incision, excess skin and fatty tissues are removed. Because the incision follows the natural contour of the upper eyelid, it will be well camouflaged when healed.

Improving the appearance of your lower eyelids

For lower eyelid surgery, often an incision is hidden just below the lower eyelashes. Through this incision, excess skin, muscle and fat are removed, or fat may be redistributed to eliminate hollowness or circles under the eyes. As in upper eyelid surgery, the incision is well camouflaged by natural creases.

In some cases, the best approach for removing excess fat may be through an incision that is placed through the inside of the eyelid. This completely avoids the need for an external incision and patients often return to work in just a few days. If a small to moderate amount of excess skin is present, laser resurfacing may be used in conjunction with this method to tighten lower eyelid skin, without any visible incision.

Forehead lifting

The effects of aging of the brow and forehead can show many different signs: drooping of the eyebrow to create a narrow space between the eyebrow and eyelashes, hooding of the eyelid skin at the outer corners, deep furrows along the forehead. These forehead and brow abnormalities may give you a tired, angry or sad expression that may not reflect how you actually feel. If this is the case, then a Forehead Lift may be the best solution. Based on variations in how men and women age and on new advances in medical technology, different methods are used to perform this procedure. Recent advances in plastic surgery have made the Forehead Lift a relatively simple procedure with results that will last from 5 to 10 years or longer. It can be done alone, or in conjunction with other procedures such as Eyelid Surgery or Laser Resurfacing.

Understanding the forehead lift

In the majority of cases, Dr. Yohai recommends the Endoscopic Forehead Lift which allows for smaller incisions, less numbness and more rapid healing following the forehead lifting. This procedure can significantly elevate the brow in most cases, without the larger incision of previous methods. The Endoscopic Forehead Lift is appropriate for both men and women. In some cases, where there is very severe brow droop, or very deep forehead furrows, then an Open Forehead lift is preferable. For some women who have a very high hairline, the incision is curved to come just to the front edge of the hairline. Thus the forehead will actually be shortened and the hairline slightly advanced. The incision is then enclosed meticulously to provide an "invisible scar," in most patients.

Laser Resurfacing

Although all of the previous surgeries can tighten and tone the face, none of the previously listed procedures can provide any significant improvement in fine wrinkles around the face. This is particularly true of fine, creepy wrinkles in the lower eyelids, crows feet and fine wrinkles around the mouth. Fortunately today, using the latest laser technology, Dr. Yohai is able to help smooth these fine lines. This laser technology gently vaporizes skin imperfections, revealing the new skin underneath. Laser Resurfacing can remove or reduce wrinkles and fine lines around the eye and mouth area, soften deep laugh and frown lines and help improve the tone of sun damaged skin. It does this by precisely removing the old surface layers of the skin, allowing a new fresher skin to emerge. As the skin heals and tightens, it will remain pink for weeks to months following surgery. This pinkness can easily be camouflaged using make-up.

During your consultation, the condition of your skin will be discussed in detail, with particular attention to wrinkling, acne scars and other changes that may be a concern to you. Choosing your surgeon

Cosmetic surgery around the eyes and face can greatly improve your appearance. The eyes, however, are among our most expressive and most fragile features. Surgery that alters the function of the brow and eyelids can have a profound effect on the health of the eyes. It is for this reason that the subspecialty of Ophthalmic Plastic Surgery came into being in the 1970s. Dr. Yohai's approach combines the artistry of a plastic surgeon with the safety inherent in being a Board Certified Eye Surgeon. It is his goal to provide improved appearance while preserving normal function.

Before you opt for surgery, consider carefully

Cosmetic surgery of the eye can make a tremendous difference in a patient's physical appearance and mental outlook, but such surgery is a result of good rapport between patient and surgeon, trust based on realistic expectations and exacting medical expertise.

In your initial consultation with Dr. Yohai, you will discuss your motivations and expectations. Your facial features will be evaluated to determine how surgery could affect your future appearance. We will take into consideration your medical history, age, skin texture, and general health. A detailed description of the procedure will also include a discussion of the risks involved, the technique indicated, the type of anesthesia, the surgical facility and the costs. We will ask you to refrain from taking aspirin or other anti-inflammatory medication for two weeks prior to surgery. If you smoke, you will be asked to stop smoking for two weeks before and after surgery. Dr. Yohai will answer all your questions during your initial consultation. If further questions emerge, please call Dr. Yohai or his staff at any time.

Your postoperative care

There may be some bruising and swelling for one to as many as six weeks following surgery. It is important that you follow the advice of your surgeon on resuming normal activities. For most patients, the recovery time will not exceed two weeks, but patients may still be advised to avoid strenuous activities for longer periods. Although most of the cosmetic changes due to the surgery occur within the first few weeks, the full effects of the surgery may not be apparent for up to a year. While the aesthetic procedures outlined here do not stop the clock, they can minimize the appearance of aging for years. As time passes, you may want to repeat the procedure. But even if you don't, you'll look better than if you never had aesthetic surgery at all.
